How to Apply for a TUE
Please follow the steps below to complete the Therapeutic Use Exemption Application. If you have any questions you may contact USADA at 719-785-2000 or by email at tue@usada.org or athleteexpress@usada.org
Step 1 - Download Appropriate Application
If your sport is listed below, you must download the appropriate sport specific TUE
Application Form. If your sport is not listed below, download the USADA Therapeutic Use Exemption Application Form

Step 2 - Download Specific Medical Information
If your application is for one of the conditions listed below, download the appropriate guidance document. If your condition is not listed below, proceed to step 3.
ADD/ADHD (stimulants) Adrenal Insufficiency Asthma (Beta-2 Agonists) (*Recently Updated*) Diabetes Mellitus (Insulin) Growth Hormone Deficiency (childhood) Growth Hormone for Idiopathic Short Stature in children Growth Hormone Deficiency (adult) Infertility (hormones)/Polycystic Ovarian Syndrome Inflammatory Bowel Disease Intravenous Infusion Musculoskeletal Injuries Narcolepsy-Cataplexy (modafinil) Post Infectious Cough Renal Transplantation, Renal Disease Sinusitis/Rhinosinusitis
Step 3 - Bring form and any guidance document to your primary care physician
Your doctor MUST fill out ALL forms correctly. Bring all forms with you to your doctor's visit.
Please
note, any TUE application submitted to USADA without medical
documentation will be returned to the athlete and will not be processed.
Step 4 - REVIEW of TUE Application before submission to USADA
Please
review your TUE application to ensure that this is a complete
application with the proper medical documentation organized in a logical
fashion. All TUE applications must contain sufficient medical
information; clinic notes and laboratory testing notes to allow the
USADA TUE Committee make the same diagnosis and arrive at the same
treatment plan as your physician without ever seeing the patient. At a
minimum, please verify that you have provided the following medical
documentation:
A)
A complete, comprehensive medical history of your diagnosis (age of
onset, severity, progression of symptoms over time etc.) and management
strategies (including a description of attempts to use non-prohibited
medications, and a clear statement from your physician why the use of
non-prohibited alternatives are not effective in symptom management).
B)
Pre-medication lab results and pertinent measurements must be provided
in addition to post-medication results (if applicable).
C)
Physicians should send copies of medical records and not just a brief
comment as to your medical condition and prescribed medication.
Step 5 - Submit COMPLETED application to USADA.
Submit application by email to tue@usada.org or by Fax 719-785-2029. For those sports with a sport specific International Federation TUE, you may submit the application to USADA and we will handle getting the information to your International Federation.
Please
note, any TUE application submitted to USADA without medical
documentation will be returned to the athlete and will not be processed.
